To Refer A Client To The Animal Medical Center |

| Your client may call The AMC Appointment Desk at (212) 838-7053 seven days a week, from 9 a.m. to 10 p.m., or |
| For personalized service, you may call our Referral Office directly at (212) 329-8890 between 9 a.m. and 5 p.m. |
| |
- As the Referring Veterinarian, you should write a short note on your letterhead listing the medical problems, relevant treatments and the reason for referral.
- Attach relevant diagnostic/lab test results and medical record copies.
- Be sure to include radiographs (labeled with your name and return address) and a copy of a current rabies vaccination certificate.
- Return all forms and records to our Referral Office Fax Line at (212) 832-5417.

To Make An Emergency Referral |

| Our Priority Emergency/Critical Care Hotline at (212) 329-8616 is a special service that The Animal Medical Center offers to Referring Veterinarians. With the hotline, a Criticalist is always available to speak to you directly, noon-midnight, 7 days a week. |
| Send your client directly in to The Animal Medical Center in an emergency, then fax your client's information to Medical Records at (212) 752-2592. |
