The Animal Medical Center, Inc. ("we" or "The AMC") is committed to protecting your privacy. Specifically, this Privacy Statement describes what information we collect through the web site we operate at amcny.org ("Our Site"), how we use it, and how you can make choices about our use of that information.
 
By using Our Site, you consent to our collection and use of information about you in accordance with this Privacy Statement. 
 
Location of Servers and Transfer of Information 
 
Our servers are located in the United States. If you are located outside of the United States, be aware that the information you provide to us will be transferred to the United States. By using our Site or providing your Personal Information (as defined below) to us, you consent to this transfer and to the collection, storage, processing and use of your Personal Information in the United States. 
 
Collection of Personal Information  
 
We collect personally identifiable information about you when you voluntarily provide it ("Personal Information"). For example, we collect Personal Information when you complete an online form. The Personal Information we collect includes your name, address, telephone number, fax number, e-mail address, job information, information about your pet and your pet's health, and information about your referring veterinarian. 

We use your Personal Information for the purposes for which you provide it, as well as to keep track of what services from The AMC you have received so that we can improve our offerings to you. For example, when you give us information about your pet's health along with your contact information, we use that information to provide veterinary care to your pet and to communicate with you and your referring veterinarian about that care. From time to time, we may provide third parties, such as our marketing partners, with Personal Information in an aggregate form that does not identify you or enable them to contact you.
 
We also use your Personal Information send you information about The AMC and our services. If you no longer wish to receive communications from us, you can follow the opt-out instructions in the materials you receive or email us at info@amcny.org. Clients may also call 212 329-8690 and veterinarians who receive our fax updates may call 212-329-8890. 
 
Collection of Donations via Groundspring.org  
 
To accept your donations to support our programs, Our Site refers you to the Groundspring.org web site. Groundspring.org is a not-for-profit organization that accepts donations on behalf of other not-for-profit organizations like AMC. Because Groundspring.org is a separate organization with a separate web site, our Privacy Statement does not apply to Groundspring.org. Rather, Groundspring.org follows its own privacy practices for the collection and use of information you provide to it via its web site. Groundspring.org makes available to us some of the information you provide when you make a donation, such as your name, contact information, the amount of your gift, the particular AMC fund you designated to benefit from your donation, and any honorary or memorial dedications (e.g. gifts "in honor of" or "in memory of" a person or pet). We treat this information just like any other Personal Information we collect via Our Site. Groundspring.org does not share your credit-card number with us. 
 
 
Anonymous Information and Cookies  
 
We may also collect anonymous information about each user of Our Site, such as the time and date of visits to Our Site, the type of web browser used, the web site that referred or linked a user to Our Site, the top search keywords that lead users to Our Site, the first page viewed by a user upon arrival to Our Site, the other pages of Our Site subsequently viewed by a user, the time spent on each page, and the user's IP address, which is the number assigned to the user's computer to identify its location on the Internet.

Persons Under Age 18
