Fill out the form below and an AMC team member will contact your client to schedule the appointment. Please do not have your clients call AMC to schedule referral appointments.

In an emergency, send your client directly to the AMC and call our 24/7 Priority Emergency/Critical Care Hotline at (212) 329-8616.

rDVM Portal

Good news! On October 15th, 2024, AMC launched a new EMR portal through Instinct, making it easier than ever to provide efficient and exceptional care to our shared patients.

To create an account, you will need to follow the link on your Instinct patient visit emails, generated when your patient checks in at AMC.
