Eligibility Application
Over a century ago, the Schwarzman Animal Medical Center was founded on the principle of providing companion animals with humane, quality care and treatment. Abiding by this mission and our commitment to serve the community, AMC has established financial assistance programs for eligible pet owners who cannot afford veterinary care and animal-related charities and rescue organizations.
For individual applicants who demonstrate financial hardship, either reduced payments or subsidized care may be available based on fund availability and medical criteria. Applicants must reside in the tri-state area (New York, New Jersey, or Connecticut) and receive a medical referral for their pets by an AMC veterinarian (this does not apply to rescue groups applying for AMC to the Rescue funds). Financial Assistance for Individuals
Application Process
The process for applying for one of AMC’s Community Funds (listed below) includes these
steps:
- Complete our Eligibility Application to determine if you qualify.
- Complete, sign, and return the financial assistance application with supporting documentation (listed on page 3 of the application) to a Client Services Manager. Applications and supporting documentation may be returned in person or emailed to funds@amcny.org.
Application Deadline
Completed applications must be received by AMC no later than 5 business days following the start of treatment. Applications received after this deadline will not be considered.
Applicant Responsibility
Applicants are encouraged to submit their application as early as possible prior to scheduled treatment. AMC recognizes that in emergency situations it may not be possible to collect all required eligibility information to submit a completed application prior to treatment. AMC cannot approve funding without a completed grant application. Applicants are responsible for all costs of treatment that is not covered by the grant amount.
Required Documentation
Your application must include copies of the documents that apply to you and anyone else residing in your household. Please submit copies, not originals, as AMC will not return any documents sent with the application. If any of the documents are missing, it will delay the processing of your application.
Applicants will be notified of their application status, via telephone, within 5 business days of receipt and review. A Client Services Manager can help you with a payment plan for any remaining charges or bills that are not covered by AMC financial assistance. AMC shall have the discretion to deny any grant application it deems inappropriate or that does not maximize benefit from the financial resources of the fund. If you have any question regarding available grants or the application process, please contact a Client Services Manager who will be happy to assist you.
Medical Criteria
At the Schwarzman Animal Medical Center, we’re dedicated to providing compassionate care to our beloved companion animals. We provide financial assistance to as many patients as possible, however our resources are limited. In all cases, we strive to make decisions that maximize our resources based on medical history, clinical status, and overall long-term prognosis.
The Buddy Fund – Cancer Treatment
The Buddy Fund was established in 2006 to provide free or subsidized care for the treatment of cancer and cancer-related conditions in animals, keeping pets healthy and with their families throughout these difficult events. The Buddy Fund is limited to one pet per client/household.
Requirements:
- Your pet must first be examined by an AMC veterinarian to determine if the pet meets the medical criteria of the fund.
- Applicants must fill out the financial assistance application and return it with all required supporting documentation no later than 5 business days following the start of treatment.
- Applicants must not have an outstanding balance.
- Applicants cannot be a recipient of another AMC grant.

Charlie’s Patient Assistance Fund – Helping those in need
Charlie’s Patient Assistance Fund helps AMC care for the pets of people who cannot afford the full cost of treatment by providing free or subsidized emergency veterinary services.
Requirements:
- Your pet must first be examined by an AMC veterinarian to determine if the pet meets the medical criteria of the fund.
- Applicants must fill out the financial assistance application and return it with all required supporting documentation no later than 5 business days following the start of treatment.
- Applicants must not have an outstanding balance.
- Applicants cannot be a recipient of another AMC grant.

Frank V. D. Lloyd Fund for Guide Dogs
The Guide Dog Program supports AMC’s visually-impaired clients by providing free veterinary care for their active guide dogs. There is no application process for this fund.
Requirements:
Guide dogs must have been trained at one of the following accredited schools:
- Fidelco Guide Dog Foundation, Inc.
- Freedom Guide Dogs for the Blind, Inc.
- Guide Dog Foundation for the Blind
- Guide Dogs for the Blind, Inc.
- Guide Dogs of Texas, Inc.
- Guide Dogs of the Desert International
- Leader Dogs for the Blind
- Minnesota Guide Dog Breeding Centre
- Pilot Dogs, Inc.
- Southeastern Guide Dogs, Inc.
- The Seeing Eye, Inc.
Additional Requirements
- At initial registration, guide dog must be accompanied by handler with ID/tags
and/or letter from one of the accredited schools above. - Handler must reside in the tri-state area – New York, New Jersey, or Connecticut.
- The Guide Dog Fund is limited to one guide dog per client.
Honey Bunny and Rou German Shepherd Fund – For owners of rabbits or German Shepherds
Established in 2013, the Honey Bunny and Rou German Shepherd Fund provides needed specialty veterinary care to rabbits and German Shepherd dogs of clients with limited financial means. The Honey Bunny and Rou German Shepherd Fund is limited to one pet per client/household per 12 month period.
Requirements:
- Your pet must first be examined by an AMC veterinarian to determine if the pet meets the medical criteria of the fund.
- Applicants must fill out the financial assistance application and return it with all required supporting documentation no later than 5 business days following the start of treatment.
- Applicants must not have an outstanding balance.
- Applicants cannot be a recipient of another AMC grant.

Kiki White Umbrella Cockatoo Avian Fund – For bird owners
Established in 2014, this fund helps subsidize the costs of specialty veterinary care for all birds owned by families with limited financial means. The Kiki White Umbrella Fund is limited to one pet per client/household per 12 month period.
Requirements:
- Your pet must first be examined by an AMC veterinarian to determine if the pet meets the medical criteria of the fund.
- Applicants must fill out the financial assistance application and return it with all required supporting documentation no later than 5 business days following the start of treatment.
- Applicants cannot be a recipient of another AMC grant.

The Monty and Minny Fund – For rehabilitation services
The Monty and Minny Fund, named after two standard poodles treated at AMC, was created to help provide rehabilitation services for pets whose owners can’t afford such care. The Monty and Minny Fund is limited to one pet per client/household per 12 month period.
Requirements:
- Your pet must first be examined by an AMC veterinarian to determine if the pet meets the medical criteria of the fund.
- Applicants must fill out the financial assistance application and return it with all required supporting documentation no later than 5 business days following the start of treatment.
- Applicants must not have an outstanding balance.

Seniors’ Animal Veterinary Effort (SAVE) – For senior citizens age 65+
The Seniors’ Animal Veterinary Effort provides free or subsidized veterinary services for the pets of senior citizens whose current financial status would qualify them to receive funding to assist with their pets’ veterinary care.
Requirements:
- Your pet must first be examined by an AMC veterinarian to determine if the pet meets the medical criteria of the fund.
- Applicants must fill out the financial assistance application and return it with all required supporting documentation no later than 5 business days following the start of treatment.
- Applicants must not have an outstanding balance.

Discounts for Military and Adopted/Rescued Pets from Partner Organizations
The Schwarzman Animal Medical Center offers the following discounts:
- Active Military: 10% discount. A valid military ID must be presented at registration to receive the discount.
- Animal Rescue Fund of The Hamptons (ARF) adopted pets: 20% discount. The blue adoption tag provided upon adoption must be presented with the pet at registration to receive the discount.
- North Shore Animal League (NSAL) adopted pets: 20% discount. The blue adoption tag provided upon adoption must be presented with the pet at registration to receive the discount.
- AMC to the Rescue adopted pets: 20% discount. The blue adoption tag provided upon adoption must be presented with the pet at registration to receive the discount.
Financial Assistance for Rescue Organizations
AMC to the Rescue
This fund was established specifically to provide subsidized specialty care to animals currently cared for by rescue groups, whose health has become an obstacle to their adoption. The Schwarzman Animal Medical Center understands the plight of those who operate rescue groups and the often limited funds available for treatment of animals beyond general care. Therefore, through AMC to the Rescue, we have provided a means for some of these needy animals to receive care. Learn more and apply.
Specialty Services Discount
The Schwarzman Animal Medical Center offers a 20% discount on services provided to animal related non-profit organizations throughout the United States. In order to receive the discount, the following is required:
- Upon the initial visit for each animal, a letter on the organization’s letterhead, signed by an officer of the organization authorizing the handler to bring the animal to AMC on behalf of the organization. This letter is required for every new animal presenting to AMC.
- The letter should accompany the handler the day of the visit or in advance of the visit.
Accounts will be set up with AMC under the name of the organization – not individuals. The 20% discount will be applied to the account. For every new animal presenting to AMC, the following is required:
- The organization must provide proof of its 501(c)(3) non-profit status in the form of an IRS letter of determination.
- Organization must not have an outstanding balance at AMC.
